What is an intake form?

This form is used to generate your secure Health Profile which your .doctor GP will have access to, as well as setting you up to allow us to bulk bill your appointment to Medicare. 

Who has access to my information?

The information your share on this form is kept confidential and will only be accessible by our clinical and support team. 

When do I have to complete this form?

You must complete this form before attending your first appointment with us. You won’t be able to attend your appointment if you don’t complete this form.

Who are the doctors?

Every .doctor GP is an AHPRA-registered & fully-insured doctor based in Australia who adheres to the highest clinical standards. Your doctor’s provider and prescriber number will be written on any prescription or referral given.

Who can use .doctor?

Anyone in Australia with a Medicare card can attend an in-person Onboarding Appointment. Only those who have attended an in-person Onboarding Appointment can book general telehealth (video or phone) appointments.

Can I change my answers later?

If you need to update your profile with new information, you can do so by contacting our Support Team directly.
